Dental Insurance Information

Your dental health is important to Kelowna Dental Centre and your health should never be compromised due to finances. Kelowna Dental Centre strives to provide the best dental services to patients and as a courtesy will gladly help you submit your claims on your behalf to your insurance provider. In order for us to submit your claim, you must first provide us with consent. Please print out and complete the attached Insurance form. You may also request a copy of this form from our Reception team.

It is your responsibility to be informed of the details of your individual contract. This contract is between you, your employer and your insurance company. We are happy to help you with answering generic questions to the best of our ability, and if you have the Insurance booklet provided to you, we will gladly review it with you to help make sense of your coverage. However, as an outside party, we are not provided directly with any information about your coverage details. If you have any specific questions regarding your insurance plan or details of coverage, we advise you to contact your Human Resources coordinator or your insurance company.

If you are not familiar with your contract, we will submit your first claim on your behalf based on an estimate of basic coverage.
Co-payment (the patient’s portion, not covered by insurance) is due at the time of treatment. Once reimbursement of services is received, you will be notified if there is a credit on your account or if a balance is due. Any uncovered amounts or unpaid claims are the responsibility of the patient to pay in full.
